POSITION SUMMARY
MDS work is where clinical accuracy and facility reimbursement meet, and most buildings feel it immediately when the seat is empty. We place contract MDS Coordinators to cover vacancies, clear assessment backlogs, correct coding accuracy problems, and train facility staff on the RAI process. Some assignments are on site, some are hybrid or remote, and some are short projects with a defined deliverable.

KEY RESPONSIBILITIES

Complete the Resident Assessment Instrument (RAI) process — MDS 3.0 assessments, CAAs, and care plan development — within CMS timeframes

Coordinate the interdisciplinary assessment schedule and ensure timely, accurate submission to CMS

Set and validate PDPM classifications and case mix to reflect the care actually delivered and documented

Audit supporting documentation across nursing, therapy, dietary, and social services to substantiate coding

Monitor Medicare eligibility, benefit periods, ARD scheduling, and required notices

Track quality measures and Five-Star clinical indicators; identify and correct coding-driven issues

Participate in Medicare and utilization review meetings

Train facility nursing and interdisciplinary staff on RAI requirements and documentation standards

Clear assessment backlogs and correct prior submissions where indicated

Support the facility during survey and payer audits on assessment-related questions
QUALIFICATIONS
Required
Preferred
-
Active Minnesota RN or LPN license in good standing
-
Minimum 2 years of MDS Coordinator experience in a skilled nursing facility

Working proficiency with MDS 3.0 and the RAI User's Manual

Working knowledge of PDPM, Medicare Part A requirements, and Minnesota case mix
-
Strong attention to detail and accurate coding across payment categories

Proficiency with PointClickCare or a comparable EHR/MDS platform
-
RAC-CT certification (AANAC) or comparable MDS certification
-
Experience clearing assessment backlogs or correcting prior submissions

Experience training facility staff on the RAI process

Multi-facility or consulting experience
CREDENTIALING REQUIREMENTS
All Caring Clinical Partners professionals must have the following on file before an assignment begins:

Current, unencumbered Minnesota license or certification in good standing

Background study clearance through the Minnesota Department of Human Services (NETStudy 2.0)

Current TB screening and immunization records

Current BLS/CPR certification (where required for the role or client facility)
INDEPENDENT CONTRACTOR TERMS

This is a 1099 independent contractor position, not employment with Caring Clinical Partners or the client facility. You control the manner and means of your professional work.

As an independent contractor, you are responsible for your own federal and state taxes, professional liability coverage, and license maintenance.

On-call days must be listed by the consultant to be paid.

Mileage is not reimbursed. Lodging may be arranged for assignments far from home.
